Supported Living Services

a. Description: The Supported Living Program will provide training in daily living skills to clients from the entire state. This training will prepare each client to become as independent as possible while living in the least restrictive environment. The training arranged by the Supported Living or Follow Along Coordinator is provided by house and apartment managers and client instructors.

Admission criteria
b. Diagnosed with mental retardation, cerebral palsy, autism or epilepsy 2. 18 years or older 3. Assessed needs of client

c. Training provided during 1. 6:00 am – 7:45 am, Monday – Friday, 365 days per year2. 3:30 pm – 10:00 pm, Monday – Friday, 365 days per year3. 7:00 am – 10:00 pm, Weekends, 365 days per year4. Other hours as needed

d. Types of training1. Daily living skills (i.e., health and safety, personal grooming, housekeeping, shopping, dressing, cooking, laundry). 2. Money management. 3. Social skills.

e. Discharge/exit criteria1. When a client has attained necessary skills for a more independent living situation which requires less supervision or support system. 2. When a client cannot function in the Supported Living Program because of incompatibility or inability to acquire skills necessary for daily functioning.

f. When or if the above criteria is met, then every effort will be made to find an appropriate living situation.

g. Transportation services are provided from and to each client’s home daily.
